By pressing the submit button I am asking J-S Training and DPPi to reserve me a place on the above stated session.  I will print a copy of this page, sign it and send it to DPPi at the address following with a cheque for the full amount of this session.  I understand that by reserving this session via the submit button a place will be held for me for SEVEN days.  If payment is not received within the SEVEN days J-S Training and DPPi are not obliged to hold a place for me on the above session.  There is no charge for holding a reservation. 

If I need to cancel my place after I have paid I will contact J-S Training or DPPi in writing as soon as possible.  If I give more than SEVEN days notice of my wish to cancel from the session date I understand that I will be liable for a £10 administration fee, all other monies paid to DPPi will be returned by cheque to the address on the booking form.  If I give less than SEVEN days notice I understand that I will still be liable for the whole amount and will not have monies returned to me.

If my organisation wishes to change the name of the delegate then please inform DPPi or J-S Training as soon as possible.  If this change occurs in the FIVE days prior to the session my organisation will be liable for a £5 administration fee

If for any reason J-S Training or DPPi have to cancel the above training session we will give you more than SEVEN days notice from the course session date.  You will be given the option of either rebooking for a different date or having all monies returned to you.  J-S Training and DPPi will not be liable for any other cost/expenses incurred due to the cancellation of this training session
